Outpatient - Neuro Rehab Residential - Home & Community
At Centered Care, we deliver return to function & purpose-drive rehab - every therapist, every patient, every session.
We understand that recovery from neurological conditions and catastrophic injuries can be a complicated journey with evolving needs throughout the recovery process. To address the evolving needs, we have designed three programs to meet individuals where they are:
Outpatient – Our outpatient facility offers therapy five days a week, from 8am to 5pm, in a welcoming atmosphere with state-of-the-art technology for patients 18 and older. Our services include:
- Physical therapy
- Occupational therapy
- Speech language pathology
- Therapeutic Recreation
- Counseling
- Music Therapy
- Vocational Rehabilitation
We know that more than anything, return to function is key for long-term success, and our services are not limited to the clinic. Although treatment usually begins in the outpatient clinic, our team welcomes opportunities to meet the people we serve in their homes or in the community to reinforce and integrate learned skills when and where they are needed. Telehealth services can be arranged on a case-by-case basis.
Neuro Rehab Residential Program - Our NRRP Therapy Program is a vibrant part of our Neuro Rehab Residential program (NRRP). Our therapy services are provided up to 5 days a week, as well as weekends and holidays. Our therapists organize special events that allow our residents to showcase their talents and skills, and connect with the community.
We strive to challenge our residents in ways that promote functional growth as well as enrich their lives as residents of the NRRP. Based on each resident's interests & needs, we have a Growth track and an Enrichment track that determines intensity of therapy services vs. gradual transition to productive activities including work, community reintegration, leisure opportunities, residential responsibilities, and other post - rehab opportunities.
Home & Community Rehab - Our dynamic team of therapists are excited to meet people where they are—literally! Treatment sessions can occur in private homes, grocery stores, libraries, shopping malls, parks, and more—wherever the skills are needed and can be applied. The Centered Care team is dedicated to building and maintaining community resources and connections that we can share with the people we serve through our Home & Community Rehab program. We provide services to within a 20-mile radius of our outpatient center. We have seen that at times people who start in our outpatient center eventually graduate to sessions based in the community that allow them to generalize their skills to functional situations, or homecare patients develop the ability to access the community and eventually take advantage of our state-of-the-art equipment in our outpatient center.
