Heather Kluemper is a Nurse and Certified Brain Injury Specialist with more than 25 years of experience in complex medical care, rehabilitation, and patient advocacy. Her career began in St. Louis, Missouri, and has since evolved into a deep and ongoing commitment to supporting both pediatric and adult patients with traumatic brain injuries (TBI), spinal cord injuries (SCI), and medically fragile conditions.
As Co-Founder of Centered Care of Michigan, Heather has played a key role in developing Joint Commission- and CARF-accredited programs that provide person-centered, skilled private duty nursing and rehabilitation. She is passionate about ensuring every individual—regardless of age or complexity of need—receives care that honors their dignity, goals, and full potential.
Heather’s experience also includes serving as Assistant Project Manager for LiveSTRONG in Louisville, Kentucky, where she led patient education and wellness retreats for individuals newly diagnosed with cancer. Her focus on whole-person, family-inclusive care has become a hallmark of her approach in every setting.
Most recently, Heather founded The Mitten Pediatric Palliative & Hospice Foundation, a nonprofit dedicated to supporting Michigan families caring for seriously ill children. Through advocacy, community partnerships, and fundraising, she is working to expand access to compassionate, skilled pediatric palliative and hospice care throughout the state.
Heather believes true care begins with listening, and that healing is most powerful when delivered with heart, hope, and humanity.

With over three decades of nursing experience, Teresa Hiatt has built a career defined by clinical excellence, compassionate leadership, and a steadfast commitment to patient-centered care. Beginning her career in Tennessee, she honed her skills on a cardiac step-down unit, gaining a strong foundation in acute medical care and critical thinking. Early on, she developed a passion for supporting patients and families through complex health challenges—a passion that would shape her entire career.
After relocating to Michigan, Teresa discovered her calling in long-term care and rehabilitation. She became instrumental in establishing a Neuro Rehabilitation Program, designed to help individuals living with traumatic brain injuries, spinal cord injuries, and other neurological conditions regain function and independence. Her leadership in this area was complemented by her deep expertise in complex wound care, where she not only treated patients but also trained and mentored other clinicians to elevate standards of care.
In addition, Teresa helped grow a strong skilled private duty division, ensuring families had access to safe, reliable, and personalized care at home. Her ability to balance operational excellence with clinical compassion quickly earned her recognition as both a trusted nurse and a respected leader.
In 2021, Teresa co-founded Centered Care of Michigan alongside fellow nurse leaders, driven by a shared vision to create a healthcare organization rooted in dignity, empathy, and person-centered rehabilitation. Today, as Chief Operating Officer, she oversees daily operations, compliance, and program development. Under her guidance, the organization has achieved Joint Commission and CARF accreditations, demonstrating its commitment to the highest levels of quality, safety, and accountability.
Beyond her administrative leadership, Teresa remains deeply connected to bedside care. She is known for her quiet dedication, her patient-first philosophy, and her ability to lead by example. Her colleagues describe her as steady, dependable, and compassionate—qualities that inspire both her team and the families she serves.
Outside of her formal roles, Teresa continues to mentor emerging healthcare professionals, advocate for vulnerable patient populations, and champion the importance of family involvement in the care journey. She brings not only clinical knowledge but also a strong belief that true healthcare is built on relationships, trust, and compassion.

Jackie Miller is a Nurse and Co-Founder of Centered Care of Michigan, where she brings over 10 years of experience in skilled nursing, home care, and complex rehabilitation. Known for her calm presence and deep compassion, Jackie has been a driving force behind Centered Care’s mission to deliver individualized, high-quality care to adults and pediatric with complex medical conditions.
Jackie’s nursing journey has always centered around empowering patients and families. She has worked across a range of healthcare environments—from private duty and home health to facility-based care—developing a strong reputation for clinical excellence and patient advocacy.
At Centered Care, Jackie has helped build programs that prioritize dignity, respect, and long-term recovery. Her leadership has been instrumental in securing accreditations from both the Joint Commission and CARF, and she continues to mentor care teams in delivering thoughtful, patient-centered support.

Dr. Erika Erlandson is a physician specializing in the care of children with complex chronic medical conditions. With expertise in Pediatric Physical Medicine and Rehabilitation and advanced training in Pediatric Palliative Care, she is uniquely equipped to support children with intricate and evolving healthcare needs. Dr. Erlandson is deeply committed to interprofessional, family-centered care that prioritizes quality of life and functional goals for children facing life-limiting and chronic illnesses. She believes in honoring each child’s individuality and is dedicated to helping them discover joy and meaning in their lives.
