Centered Care, LLC operates specialized Neuro Rehabilitation Residential Program homes licensed by the State of Michigan as Adult Foster Care Homes. We are proud to hold dual accreditation from both The Joint Commission and CARF, demonstrating our commitment to the highest standards of safety, quality, and excellence in rehabilitation care.

Located in the peaceful, scenic community of DeWitt, our homes provide a clean, safe, and nurturing environment that fosters healing and recovery. Each residence features barrier-free indoor and outdoor spaces, specifically designed to encourage independence and active participation throughout the rehabilitation process. 

Our homes are structured to support meaningful engagement in both household and recreational activities of daily living. With low staff-to-resident ratios and wheelchair-accessible transportation, we are equipped to assist individuals in attending essential appointments and participating in community reintegration, vocational training, and recreational outings.
