Centered Care, LLC is committed to delivering exceptional, holistic care through our comprehensive network of neuro-rehabilitation programs, therapy services, and home care support. Our mission is to provide nursing-centered, person-centered, and hope-centered care that upholds the dignity, autonomy, and individual goals of every person we serve.

We specialize in supporting person served across the entire age spectrum—from infant to older adults—navigating a wide variety of complex health challenges, including neurological conditions, injuries, chronic illnesses, and developmental or physical disabilities. Whether in a structured rehabilitation setting or the comfort of a person’s home, our interdisciplinary teams work collaboratively to promote recovery, independence, and an enhanced quality of life.

Overview of Services Offered:

Centered Care, LLC provides the following services:

  • 24/7 Residential care with Supervision.
  • 1:1 Skilled Neuro Rehab Tech/CNA/Home Health Aide for up to 24 hours daily for ADLs, Supervision and cueing if needed.
  • Nursing: Skilled Nursing assessments as needed for change in condition, Nurse to attend medical appointments with persons served, Daily Skilled Nursing Assessments PRN, up to 24/7 Skilled Nursing
  • Rehabilitation services, including: Physical Therapy, Occupational Therapy, Music Therapy, Recreational Therapy, Vocational Rehabilitation Services, Counseling Services, Dietician Consultation, Speech Language Pathology provided by outside providers and by referral, Behavioral Therapy provided by outside providers and by referral.
  • Palliative and Hospice Services: Provided by outside providers, via referral.

Neuro Residential rehabilitation program

Our residential program is a vibrant and diverse community of homes in the North Lansing and DeWitt area. We serve people who are 18 years of age or older and who have a neurological diagnosis or physical handicap requiring assistance with daily living tasks. Our program participants often need extensive physical assistance with transfers, bed mobility, and other activities of daily living such as eating, bathing, and dressing. Many of our residents require a mechanical Hoyer-lift for transfers and use powered wheelchairs for mobility. The people we serve often work in supported settings and receive rehabilitation services that are integrated into the community. We focus our care on the individualized goals of the people and families we serve.

Our residents must be medically stable upon admission and not require the use of a ventilator or tracheostomy for airway management, nor require 24-hour skilled nursing services.

Our NeuroRehabilitation staff are trained to promote carryover of the cognitive retraining and behavioral programming that our rehabilitation specialists and collaborators initiate. We pride ourselves on fostering a safe and supportive community and carefully screen our residential applicants to ensure that we can meet the specific needs of the applicant and are aware of any history of violent or destructive behaviors, or other limiting factors that may limit success in our program.

Home Care Services

Centered Care provides care for all persons served from pediatric to geriatrics with neonates and obstetrical persons served on a case-by-case basis

  • The person served must reside within Centered Care service area
  • Centered Care, LLC provides care for complex medical and surgical persons served. We specialize in Pediatrics, TBI and SCI requiring nursing care for at least 4 hours up to 24/7 with a licensed nurse that is trained to give total care with ADL’S, IADLS and provide nursing skills for complex medical procedures including but not limited to:
    • Ventilator dependent persons served
    • Trach dependence
    • Medication Administration via IV as approved by CNO and prescribed by physician. Please see the Medication Administration Policy for limitations on medications that can be administered in the home.
    • Peg tube care and management
    • Administration of enteral feedings via/G/J tube m. Wound care management
    • Provide ongoing patient and family centered care planning
  • Centered Care, LLC provides personal care services for persons in the home of all ages requiring care for at least 8 hours up to 24/7 with a qualified trained Home Health aide or Certified Nursing Assistant that is trained to perform the following services under the direction of a Licensed Nurse
    • Assist with all ADL’S and IADL’S
    • Personal Care
    • Meal Prep
    • Respite Care
    • Housekeeping
    • Companionship
    • Med Reminders
    • Errands and Transportation
    • Care Management
    • Any higher level acuity tasks that person served requests from aide must be pre-approved and aide will be given separate training and skills from a Licensed Nurse and competency assessed before performing (this will be done on a case by case basis)

Outpatient Services

Centered Care Therapy Services provides rehabilitation services to community members in the Mid-Michigan area. We serve people age 18 and older, with a primary focus on individuals diagnosed with neurological conditions, including but not limited to stroke, traumatic brain injury, multiple sclerosis, and Parkinson’s disease. For prospective participants with spinal cord injuries, both incomplete and complete injuries are eligible. Entry for those with injuries at level C1 to C4 are determined on a case-by-case basis. The people we serve in our outpatient clinic must be medically stable, free from acute infection or fever, and not be ventilator dependent. Services for respiratory support must be arranged by the person served and be available at the clinic during treatment when symptoms of respiratory distress may occur.

Centered Care Therapy Services truly seeks to provide care that has the goals of the people served at its heart. All participants who receive services at our outpatient clinic must be free from a history of dangerous or destructive behaviors or have the support of an active behavior management program. We ask that all our participants actively participate in treatment planning and goal formation.
